How is Hospice different?
The hospice is a home when home is no longer possible or practical, making the last days or a person’s life together with their family peaceful, comfortable and meaningful. Carefor Hospice Cornwall is a 10 private room facility
We provide compassionate, end of life care for individuals who prefer not to be in a hospital and can no longer stay at home.
Carefor Hospice Cornwall services are totally non-denominational and available free of charge to all people regardless of age, gender, race or religion.
Professional care is provided 24 hours a day, 7 days a week. Families and caregivers are encouraged and welcomed to participate in the life and care of the patient at hospice.

Residential Care Admission Process
Anyone can make a referral – patient, family, health care professional, family, friend
Each application will be reviewed to ensure that the patient meets the hospice’s admission criteria and to identify any potential supplies, equipment or education that will be required to safely care for the patient.
A home visit may be required to assess and discuss if admission to hospice is appropriate.
Criteria for Admission to Hospice
• Full awareness of diagnosis and prognosis and acceptance of the philosophy of hospice care.
• Valid Ontario health Card
• Singed Do Not Resuscitate order
• Eligibility for services through CCAC
• Durable power of attorney for personal care and for property
Priority is given to:
• Individuals who are at home and live alone
• Individuals whose care cannot be managed at home
• Hospital in patients unable to return home and who wish end of life care within the hospice
• Those with pain and symptom issues that can be managed at hospice.
